Formational Ventures
Step 1
Complete
the Account Application. There are two items combined in one document
that must be completed.
Formational
Ventures Subscribers Agreement
- Review,
complete and sign this agreement and be certain to indicate the Subscription
Investment Plan that you want.
All Formational
Ventures Members must be "Accredited Investors" under the U.S.
Security Act of 1933, Reg. D, Rule 501 (a).
Step 2
Send
this document with a check paid to "Formational Ventures, LLC".
The check amount includes the subscription fee, funds to invest in new
ventures and any associated investment expense fees.
See the Investment Plan Guidelines for the amount necessary to open
your account.
Wire Details
Also,
except for the annual subscription fee, these funds are yours and represent
your "money on account at Formational" and therefore are refundable
to you upon request.
